Visits are paid for on the date of service with the credit card stored in your profile. A superbill can be provided to submit to insurance for reimbursement. You are able to use an HSA/FSA card for payment. Labs and medications can continue to be submitted to insurance by the appropriate lab/pharmacy, respectively. Additionally, cash-pay lab options are available for those without insurance coverage.

    Pricing

Please see the online schedule for visit pricing, prices are listed after the name of the visit type. You will see two follow up visit options, one of which is shorter and less expensive for those who have a succinct medical need such as a medication refill or an acute infection. If your needs exceed this visit you will be asked to schedule another appointment. Most patients will require the longer follow up visit. There are options for in person or video appointments. For potential clients outside of Oregon video appointments are available, but please note that the scope of care is different out of state. Please inquire by phone.

    New Patients

All new patients should first schedule a New Patient Visit (in person or video), regardless of the reason for your visit. For example, if you would like to pursue pellet insertion, a new patient visit should be scheduled first. This ensures that your provider has your full medical history and can determine the best treatment plan moving forward. Feel free to schedule the free 10 minute phone call if you have questions about the services offered.

If you do not see an opening on the schedule that fits your timeline, or you have a concern that requires a visit sooner than there is an opening, please send a message to your provider or call the office and leave a message. We can almost always accommodate you within 1-2 weeks. If you have an urgent health concern, please go to an urgent care center.
